Nevermind, I just answered my own question about the ns16. I found it in the code. I'm now curious why this one report is listing the xlmns urls differently and if that could be causing the problem? I have no clue where else to look for this error message.

I'd check the contents of the XML for single quotes or double quotes in the content (between the XML tags).
