@erictgrubaugh My test account, which is working correctly, has it set to “Update the most recently modified record” and has duplicate detection enabled. It must be the way the client has their dup duplicate detection setup.

Yes if "Duplicate Detection" is checked, then it's using the company-wide duplicate detection rules
You can set up online forms to use their own duplicate detection rules, though, which is why you'll need to check how the form is set up in their account as well

Feels like it stoped working correctly when 2018.1 was released, but could of been someone playing around.
