The Show Message State is blank and on the Transition On (Tried Entry & Before Record Load) I have conditions to check the entity value. However, it seems it always goes to the first one which is Entity: 34960. It seems like it’s ignoring the Transition On condition. See any issues?

Well, 34960 is your entry state, so it has to go there first. And there is no way for it to go from that state to any other state
