Does anyone know the permissions required for editing a Custom Segment on an Item record. I have a role that can access and edit a Custom segment on transactions and Entity records but not on Items and I can't figure out why.

Have you checked the form level permissions? If it's not the form permissions, there may be a script or workflow preventing you from interacting with that field.
