This is going to take some troubleshooting by you, but the two things I can see are 1) you don't have a second else, so what happens in the case when neither of your conditions are met? and 2) I don't see anything evaluating the "latest date", so if you have multiple instances of these things happening in the system notes it's going to evaluate differently for each line. You need an Aggregate function or summary selection to do that.
