The execution contexts on the deployment are usually a good one to check

UserEvents also do not chain, so if a different user event edited/created whatever you are dealing with, they will not chain.

Thanks for your inputs. I tried to trigger the script 3 times, and I was successful in all of them. Not quite sure now what is causing the issue.
