I have. It turns out @tuvi was right. The accounts must be turned on plus, if the account is set to USD and you book a foreign currency to that account it will only revalue the FX amount as of the transaction date. It will not revalue. Therefore, you must have separate accounts for each FX or make sure the account has a zero balance.
