Anyone recently encounted an error where the workflows duplicated itself during transitions and therefore entering the state twice?

I've encountered this but it was because I didn't know about the DO NOT EXIT WORKFLOW checkbox. Not sure if that's what's causing it here.

thanks @blue! but this isn't the case this time... hmm, it isn't just one record too, it's like a few of the records we have had duplicated spontaneous concurrent workflow states.. it's a problem because we have script that run on entry, and when it happens, it runs twice, and it's stuff like transform record... ouch.
