Anyone ever see NetSuite randomly delete an integration record? It's been being used for almost 5 years. But we randomly started having issues with Boomi connecting to us through its integration record using TBA. I went to check the record.. And it says it's gone.. Just going to create a new one. But curious if anyone has ever seen this before?

No, but assume you've done a system note search on record type integration to see if a user deleted it?

Haven't seen it, but this would 100% prompt me to do a thorough role audit to see which role(s) other than administrator have access to delete an integration. If the answer is not zero, I'd take a hard look why anyone else has that access and if it's truly needed. If it's this integration user or some other integration, I'd still give serious thought to revoking this access. You should test of course, but I don't see a reason why an integration would need that either, despite what the setup instructions might say. Many apps/companies are lazy about figuring out which perms they actually need versus just saying they "need" a bunch of stuff because they don't want to take the time to figure out how things interact.

It might be if it is part of a bundle. Did you try running a Deleted Record search to see who might have done it?
