Formulas are your friend here. I take it that this is not an Item search, perhaps a transaction search that's doing a join from the Item record? Below are examples of simple formulas from a transaction search where it joins the Item: Name from your example.

It is a Tran search joining the item. What I posted is the criteria of the search. No formulas involved.

My guess is you had filters on the search - and then someone created another search from the first one. So it included the filters as criteria
