Is there a best practice way to remove unwanted extensions that have not been activated? Can we just remove the record from SC ExtMech Extension List records? Should we remove the files as well?

Make sure the extension is not activated, remove the SC ExtMech record associated, then in the file cabinet remove the folder with all the files from the deployed_extensions. I think that should keep clean your NetSuite instance.
