@Kearobi Try re-triggering the Extensions installer Map/Reduce. In the edit page of a Map/Reduce deployment you can "Save & execute" that triggers the map/reduce and it will try to process all the pending job of the type that it knows about
You can also try to change the priority of each map/reduce from Standard to High

We had the sandbox refreshed and now there are other errors present. The activation never hits pending and is showing this error now.

@Flo, thank you for the response. I did try clearing the browser cache, to no avail. Though we upgraded the Extension Managment Bundle to 2019.1 and the issue was resolved.
