first recommendation is to put all that html into a file that you load using N/file

its a huge pain to try to maintain html as a bunch of strings
use your favorite template library to fill in things like
crrUsrName
or your select options
your particular problem is probably the use of format.parse
get familiar with how Date Formats work in NetSuite
format.parse is designed to parse strings which match the current user's format
the date string from your date input probably doesnt match the format of the netsuite user
