Not that I'm aware of, since it isn't a different form as a whole but seems to be a different version of the form.

When I create a Canada order in the UI, the address pop-up reloads after I change the country, and the whole SO entry page reloads after I save the address

but it's still using the same custom form

I wonder if prior to creating the SO, it would let you just save the Canadian address to the customer record directly as an alternate address. Then when you are creating the SO, just reference the existing address in the shipaddresslist field.
