I wanted to know if there was a way to create a duplicate Custom Entry Form. Copy to account feature does not work for me.

Would the "Save As" button work? Edit the form you want to copy and hover over the save button to access Save As. Make sure you just change the name to something else that way you can differentiate between the 2.
