Is it possible to add a field that shows the user who lastly modified the form (address form) ?

Isn’t that in the history/Audit trail on the form?

There is a history tab on the address form. It shows date/time, user and type (change). so you can tell who last modified and when, just not what they did

Thanks for the comments, the customer wants to see the modifier as a field.
