Has anyone ever encountered tax collected being drastically less than tax due? What would be the issue there?

What tool are you using for tax? Is it possible you've extended a discount to the customer that's either not set up properly for tax or which is actually not reducing the tax due under that particular tax code?

Looking at Sales Tax Liability by Tax Item report - I'm not real familiar with this. But the Tax Collected is much less than the Tax Due column and I need to know why
How is that tax due column calculated?

there is a description of the report in the help menu...have you looked?
Tax Collected – this column shows the tax amount that results from multiplying the amount in the Taxable Sales column by the Tax Rate for that transaction. • Tax Due – this column reports the total tax amount due to the tax agency for this tax code. This amount is the total as of the last date selected on this report.
so it appears that tax collected is for a transaction while tax due is a running balance
